Inkscape's icon theme has not changed (it still uses the "old" colorful icons by default) - did you maybe install a custom icon theme for Inkscape in "~/.config/inkscape/icons/"? Or possibly your current desktop icon theme does provide custom icons for Inkscape …

Attaching two PDFs - one saved as copy with Inkscape 0.48.2 for Mac OS X (includes cairo 1.10), and one converted with the command line tool 'rsvg-convert' using current stable cairo 1.14.4 (we known that the command line tool is not affected by the problem which is triggered with Inkscape).

Unless you need it preserved transforms for other compelling reasons, change back to 'optimized' transforms (you likely changed the prefs [*] for Transforms from 'Optimized' (default) to 'Preserved' at some point).

[*] Edit > Preferences > Behavior > Transforms: Store transformation

I don't ever get the dialog to choose X11 (I've also installed XQuartz, and attempted to start Inkscape with/without XQuartz running first) The dialog prompting "Where is X11" only happens with older packages (Inkscape 0.48.5). The current package for Inkscape 0.91 knows to look for XQuar...
